I understand that participation in boating, fishing, paddleboarding, swimming, and other water-based activities offered by Eagle 6 Adventures ("E6A") involves inherent risks, including but not limited to: drowning, falls, collisions, marine life encounters, weather-related hazards, equipment failure, and injury arising from the use of boats, paddleboards, fishing equipment, and related gear. I voluntarily assume all such risks, known and unknown, for myself and/or the minor participant named below.
In consideration for being permitted to participate in Eagle 6 Adventures activities at no cost, I, on behalf of myself, my heirs, and my assigns, release and hold harmless Eagle 6 Adventures, its officers, directors, volunteers, and Stacey Corn individually, from any and all claims, demands, or causes of action arising from my participation, except for claims arising from gross negligence or intentional misconduct.
I represent that I (or the participant named below) am physically fit to participate, or have disclosed any relevant medical conditions to E6A staff prior to the trip. In the event of an injury or medical emergency, I authorize E6A staff to arrange for emergency medical treatment and agree that I am responsible for any associated costs.
I grant Eagle 6 Adventures permission to photograph or video the trip and to use those images for the organization's nonprofit mission, including its website, social media, and fundraising materials, unless I notify E6A in writing that I decline this permission.
By typing my name below and checking the box, I acknowledge that I have read and understood this waiver in its entirety, that I am at least 18 years of age (or am the parent/legal guardian of the minor participant named below), and that my typed name constitutes my legal electronic signature, equivalent to a handwritten signature.
